How do you collect evidence in an ISO 45001 audit?

Who should you interview in an ISO 45001 audit?

Interview people at several levels, including frontline workers, supervisors, managers and contractors, not only the OH&S manager.

ISO 45001:2018 clause 5.4 requires processes for consultation and participation of workers, so what workers say is direct evidence about whether that process works. A good interview sample covers different shifts, tasks and employers on site, and it includes at least some people who face the highest risks.

How do you prepare for a worker interview?

Prepare by understanding the process, the requirements and the worker’s role, and write prompts rather than a rigid script.

Before each interview, be clear on 3 things: the process you are auditing, the requirements that apply to it, and the person’s role in it. Then write a few prompts, not a long checklist. How do you run the interview?

Open with a clear purpose, ask open questions, listen more than you talk and avoid leading questions.

  1. Introduce yourself and explain why you are there and how the information will be used.
  2. Start with the worker’s own job, so the conversation feels natural.
  3. Ask open questions that start with “walk me through”, “what happens when”, “how do you know” or “show me”.
  4. Listen. Use short prompts such as “tell me more about that”.
  5. Close politely and say what happens next.

How do you verify what workers tell you?

Check each statement against a second source, such as a record, an observation or another person.

An interview statement on its own is a lead, not proof. If a worker says they report hazards to their supervisor, ask to see the hazard log or the follow-up. If an operator describes the lockout steps, watch the task or compare with the procedure. Using 2 or more sources before you conclude is how you move from anecdote to evidence. How do you record evidence?

Record specifics: who you spoke to, what they said or showed you, the document, the date and the place.

ISO 19011 guidance says audit evidence that leads to findings should be recorded. Capture facts, not impressions. “The operator said the guard was removed for cleaning on Tuesday, and the guard was missing at the line” is evidence. “The worker seemed unhappy” is not. Good notes make your findings easy to write. What mistakes should auditors avoid in worker interviews?

The biggest mistakes are leading questions, talking too much and treating one comment as proof.

  • Leading questions. “You do have a procedure, don’t you?” invites a yes.
  • Interviewing only managers. They describe the system as designed, not as lived.
  • Talking too much. The auditor’s job is to listen, not to explain the standard.
  • Interviewing in front of a boss. Workers may hold back, so find a way to speak with them privately.
  • Not verifying. One comment is not a finding until you have corroborated it.

Interview simulation: choose the best response

For practice only. Read each situation and choose the best response. These scenarios were written by 3FOLD Training and are not real exam questions.

Situation 1. You want to learn how hazards are reported on the packing line. Which opening question is best?
Model answer: “Walk me through what you do when you spot a hazard.” It is open, neutral and asks for a description of real practice. The other two are leading or closed (clause 5.4 consultation and participation).
Situation 2. A worker says, “We always wear the correct gloves.” A few minutes earlier you saw a colleague at the cutting station without gloves. What do you do next?
Model answer: Check the statement against other sources: records, observation and another worker. One conflicting observation is a lead, and you need verified evidence before you write a finding.
Situation 3. A supervisor stands next to a machine operator during your interview, and the operator gives very short answers. What is the best action?
Model answer: Politely ask for time alone with the worker and explain why. Workers often speak more freely without their supervisor present, and that gives you better evidence.
Situation 4. You finish an interview with a warehouse worker. What should your notes contain?
Model answer: Specific facts: who, role, what was said or shown, the document or area and the place. Specific notes make findings traceable, while impressions cannot be verified.
