ISO 45001:2018 audit focus

What auditors focus on when auditing ISO 45001:2018

ISO 45001:2018 remains the current, unrevised edition of the OH&S management system standard. So this ISO 45001 Lead Auditor training does not chase a new edition. Instead, it drills into the clauses where auditors most often find weak or missing evidence. Select a clause to see the audit focus, and then see where you practice it in this ISO 45001 Lead Auditor training.

High audit focus Moderate focus Foundational
4
Foundational

Context, and interested parties who are workers

What auditors check

  • ISO 45001:2018 makes workers, and worker representatives where they exist, an explicit interested party — not an afterthought under "other stakeholders".
  • The scope must reflect the activities, products and services under the organization's control or influence, including outsourced processes.

What it means for the auditor

  • Sample how the organization actually determined workers' OH&S needs and expectations, rather than accepting a generic issues list.
  • Check that the scope statement matches what you see on site or in remote work arrangements.

Practiced in Lesson 1 · foundations and audit checklist building

5.4
Commonly missed

Consultation and participation of workers

What auditors check

  • Consultation is two-way dialogue before a decision. Participation is workers actually contributing to hazard identification, risk assessment and incident investigation, not merely receiving information.
  • A noticeboard, an email, or a policy poster is communication (clause 7.4) — evidence of consultation and participation looks different.

What it means for the auditor

  • Interview workers directly, away from their manager where possible, and test whether they recognize being consulted, not just informed.
  • This is the clause most likely to expose a gap between a well-written OH&S manual and what actually happens on the floor.

Practiced in Lesson 2 · audit interviews and auditor competence

6
Commonly missed

Hazard identification, risk assessment and legal requirements

What auditors check

  • Clause 6.1.2.1 requires hazard identification to be ongoing and proactive — before change, after incidents, and for routine and non-routine activities alike.
  • Clause 6.1.3 requires a process to identify and keep current with legal and other OH&S requirements, then evaluate compliance against them.

What it means for the auditor

  • Trace whether a recent incident, near-miss, or change actually fed back into the hazard register — a register that never changes is a red flag.
  • Test the legal register against a live regulatory requirement, rather than accepting that a register exists.

Practiced throughout the Stage 1 and Stage 2 audit simulation

7
Evidence-heavy

Competence, awareness and communication

What auditors check

  • Competence (7.2) must reflect the hazards and risks people are actually exposed to in their role, not only a generic induction record.
  • Awareness (7.3) covers workers' own OH&S risks, their contribution to effectiveness, and the consequences of deviating from procedures.

What it means for the auditor

  • Sample people at the point of work, rather than reading a training matrix alone.
  • Ask what happens if a procedure is not followed — a confident, specific answer is stronger evidence than a signed induction form.

Practiced in Lesson 3 · nonconformity reporting and audit planning

8
Commonly missed

Hierarchy of controls, management of change and contractors

What auditors check

  • Clause 8.1.2 requires controls to be selected using the hierarchy of controls — elimination and substitution first, PPE last — not PPE as the default answer.
  • Clause 8.1.3 requires a documented process for managing change, including new equipment, processes and personnel changes that affect OH&S. Clause 8.1.4 covers procurement and contractors, including their OH&S performance while working on site.

What it means for the auditor

  • Follow one recent change, or one contractor, from induction through to on-site control and look for the evidence at each step.
  • Where PPE is the only control offered for a hazard, ask what was considered and ruled out higher up the hierarchy.

Practiced in Lesson 4 · Stage 2 audit role-play simulations

9
Evidence-heavy

Monitoring, internal audit and management review

What auditors check

  • Clause 9.1.2 requires evaluation of compliance with legal and other requirements, and clause 9.2 requires internal audits with defined objectives, not just scope and criteria.
  • Management review (9.3) must cover OH&S performance, incident trends, and the status of actions from previous reviews.

What it means for the auditor

  • Trace an internal audit finding through to a management review input and out the other side to a documented decision.
  • Test whether monitoring data actually drives a decision, rather than sitting in a spreadsheet nobody reviews.

Practiced in Lesson 5 · reporting, follow-up and closing meetings

10
Commonly missed

Incident investigation, nonconformity and corrective action

What auditors check

  • Clause 10.2 requires the organization to determine the cause, or causes, of an incident or nonconformity — not just the immediate corrective action taken.
  • Corrective action must be evaluated for the need for action to eliminate the cause, so that the same incident does not recur elsewhere on site.

What it means for the auditor

  • For any incident report, look specifically for root-cause analysis, not only "what was done afterward".
  • This is the clause the mini-audit below tests directly.

Practiced in Lesson 5 · corrective action review and exam-readiness

Already completed a CQI IRCA Certified ISO 45001 OHSMS Lead Auditor (or Auditor) training course? First answer two questions, and then we will point you to the right option.

Question 1 of 2

Have you completed a CQI IRCA certified ISO 45001 OHSMS Lead Auditor (or Auditor) training course?

What do you need from the training?

Our recommendation

You may already meet the training requirement.

You hold a CQI IRCA certified ISO 45001:2018 OHSMS Lead Auditor certificate, and ISO 45001:2018 remains the current edition of the standard. Therefore, you do not need to repeat this 40-hour course to apply for, or keep, CQI IRCA OHSMS auditor membership. Talk to an advisor about your application and audit-experience log.

Your earlier Lead Auditor training was on OHSAS 18001 or a preceding standard, which CQI IRCA does not accept for OHSMS auditor grade membership. Therefore, you need the full CQI IRCA certified Lead Auditor course on ISO 45001:2018, the current edition.

What you will learn in the ISO 45001:2018 OHSMS Lead Auditor online course

This ISO 45001 Lead Auditor training follows one certification audit from start to finish, using accelerated-learning techniques and role-play so that 80% of the course is doing, rather than listening. Lesson 1 · 1Sessions 1–3

Foundations — ISO 45001:2018 OHSMS & auditing principles

You understand the structure and key requirements of ISO 45001:2018 — organizational context, hazard identification, risk assessment, OH&S policies and objectives — and the ISO 19011 auditing guidelines. Hands-on, you audit OH&S policies and objectives and build an audit checklist based on ISO 45001:2018.

  • 1Determining which ISO 45001:2018 requirements apply to the case-study scope
  • 2Auditing OH&S policies and objectives
  • 3Building an audit checklist based on ISO 45001:2018

Lesson 2 · 2Sessions 4–5

  • 5Auditing top management in a simulated interview
  • 6Practical tips for audit preparation and execution

Lesson 3 · 3Sessions 6–7

Nonconformity reporting & audit planning

You write defensible nonconformity reports, manage corrective actions and prepare audit reports. Then you plan and conduct a Stage 1 audit and transition to Stage 2 planning — including a full Stage 1 audit of the case-study company and group exercises in audit program management.

Exam domain 4
  • 7Writing nonconformity reports and managing corrective actions
  • 8Preparing audit reports
  • 9Planning and conducting a Stage 1 audit of the case-study organization
  • 10Group exercises in audit program management; transitioning to Stage 2 planning

Lesson 4 · 4Sessions 8–10

Conducting Stage 2 audits — role-play simulations

You experience real-world auditing: lead opening meetings and execute a Stage 2 certification audit against ISO 45001:2018 requirements in breakout-room simulations, interviewing auditees, gathering objective evidence, and reinforcing nonconformity-report writing and corrective-action follow-up.

Think like a lead auditor: spot the nonconformity

Eighty percent of this ISO 45001 Lead Auditor training is doing, rather than listening. For example, here are three situations from a certification audit against ISO 45001:2018. No sign-up, no score saved.

In the live course, these become full role-plays in Zoom breakout rooms. In addition, your trainer plays the auditee and gives written feedback on every activity.

Audit evidence notesFinding 1 of 3
Evidence · Worker participationThe HSE manager shows you the OH&S policy and a completed hazard register. When you ask three workers on the shop floor whether they were consulted on the latest risk assessment for the packing line, all three say they first saw it pinned on the noticeboard.

Which requirement of ISO 45001:2018 is this evidence most relevant to?

Consultation and participation are not the same as communication. Clause 5.4 requires two-way dialogue before a decision, and workers actually contributing to the risk assessment. Posting a completed document on a noticeboard is communication (7.4), which happens after the fact.
Evidence · Hazard identificationThree weeks ago, a forklift near-missed a pedestrian at a warehouse intersection. You find the incident report and a completed corrective action to add mirrors and floor markings. However, the hazard identification and risk assessment register for that area still shows the same risk rating recorded 18 months ago, with no update after the near-miss.

Which nonconformity statement would stand up at the closing meeting?

Requirement, evidence, nature of the nonconformity. A defensible finding states all three and nothing more. The near-miss was investigated and a control was added — the actual gap is that nobody fed the new information back into the hazard register, which is precisely what 6.1.2.1 requires on an ongoing basis.
Evidence · Incident investigationYou review an incident report for a minor hand laceration. It records first aid given, the injured worker's name, and "corrective action: reminded operator to wear gloves." There is no record of why gloves were not being worn, whether the task's risk assessment addressed hand protection, or whether the same task is performed elsewhere on site.

What should the auditor do next?

One thin report is a lead, not yet a finding. Clause 10.2 requires the organization to determine the cause, or causes, of an incident — not just record the immediate action. Every incident is in scope, however minor. The auditor verifies with further sampling before concluding. The course trains exactly that judgment.

ISO 45001 Lead Auditor course — FAQ

Straight answers about ISO 45001 Lead Auditor training: approval, the exam and what your certificate means.

    Ask your own question

What is the ISO 45001 Lead Auditor course?

It is a CQI IRCA certified training course that gives you the knowledge and skills to plan, conduct, report and follow up first-, second- and third-party audits of an occupational health and safety management system (OHSMS) against ISO 45001:2018, in accordance with ISO 19011.

Is this ISO 45001 Lead Auditor course CQI IRCA approved?

Yes. 3FOLD Training is a CQI IRCA Approved Training Partner, and this ISO 45001:2018 OHSMS Lead Auditor course is certified by CQI and IRCA, UK under course reference PR357. You can verify 3FOLD on the CQI IRCA training partner directory.

Who should attend ISO 45001 Lead Auditor training?

HSE managers, safety officers, internal auditors and management-system professionals moving into OHSMS auditing; consultants; and anyone pursuing IRCA OHSMS auditor certification. CQI IRCA expects a basic understanding of ISO 45001, so 3FOLD's pre-course lessons help you get there.

What is included in ISO 45001 Lead Auditor training?

40 hours of live Virtual Instructor Led Training on Zoom, a full audit simulation on a realistic case study, pre-course modules and a prior-knowledge quiz, an activity workbook and mock exams, your first CQI IRCA exam attempt, and 3-month LMS access with recorded sessions and bonus webinars.

How long is ISO 45001 Lead Auditor training and how is it delivered?

It is a 40-hour course delivered 100% online (Virtual Instructor-Led Training). Choose a weekend batch of five Sundays (9:00 am–5:00 pm GST) or a weekday evening batch of 13 sessions (7:00 pm–10:00 pm GST). We can also schedule corporate batches around your team.

Is ISO 45001 Lead Auditor training online?

Yes. The whole course is online, including the final CQI IRCA exam, so you can complete it from anywhere. Sessions are live and interactive, with Zoom breakout-room role-plays and a trainer who plays the auditee — it is not a self-paced video course.

What is the ISO 45001 Lead Auditor course fee?

The course fee is AED 2020 or USD 550, inclusive of VAT, and includes your first CQI IRCA exam attempt. You can pay securely online in AED or USD.

How do I enroll in ISO 45001 Lead Auditor training?

Choose your batch in the schedule on this page, then pay securely online in AED or USD. A member of the 3FOLD team contacts you within 24 hours with your schedule, Zoom joining instructions and LMS access. You can also reserve a seat or ask questions on WhatsApp at +971 50 481 9989.

Do I need prior knowledge before the ISO 45001 Lead Auditor course?

No — only a basic understanding of ISO 45001 and occupational health and safety management principles is expected. 3FOLD includes pre-course modules and a prior-knowledge quiz so complete beginners can prepare before the live sessions begin.

I hold an OHSAS 18001 Lead Auditor certificate. Do I need this course?

Yes. ISO 45001:2018 replaced OHSAS 18001 as the OH&S management system standard, and CQI IRCA does not accept OHSAS 18001-based training for current OHSMS auditor grade membership. You need the full CQI IRCA certified Lead Auditor course on ISO 45001:2018.

How is the CQI IRCA exam structured and what is the pass mark?

The exam has 40 questions worth 80 marks across five domains. You must score at least the minimum in every domain and at least 50% overall (40 of 80). It is taken online within 30 days of the course; duration is 1 hour 45 minutes, extended to 2 hours 15 minutes for non-native English speakers (3FOLD applies this automatically).

Is the ISO 45001 exam open book?

Yes. You may use your course notes and a clean copy of the ISO standards. Internet access and unauthorized items are strictly prohibited and may be treated as malpractice, even if unused.

What happens if I fail or miss the ISO 45001 exam?

You can resit for a AED 125 / USD 35 fee within one year, without re-attending the course. If you also fail the second attempt, CQI IRCA requires you to re-attend the course (free at 3FOLD within one year); the levy plus exam fee for that attempt is AED 310 / USD 85.

What certificate will I receive from the ISO 45001 Lead Auditor course?

Learners who pass the continuous assessment and the CQI IRCA examination receive a Certificate of Achievement, issued digitally within 48 hours of the result; those who attend but do not pass receive a Certificate of Attendance. Only the Certificate of Achievement qualifies you to apply for CQI IRCA auditor membership.

Does the ISO 45001 Lead Auditor course make me a certified lead auditor?

Not by itself. It satisfies the training requirement for initial certification as an IRCA OHSMS auditor, provided you apply within five years of completing it. CQI IRCA also requires audit experience and assesses your application before awarding the auditor grade.

Does the ISO 45001 Lead Auditor course cover first, second and third-party audits?

Yes. A first-party audit is an internal audit an organization performs on itself; a second-party audit is performed on a supplier by, or on behalf of, a customer; a third-party audit is performed by an independent certification body to certify the OHSMS against ISO 45001.
