An emergency plan on a shelf protects nobody. What matters is whether workers, contractors and visitors know what to do when something goes wrong, and whether the organization has tested its plan and learned from it. Clause 8.2 of ISO 45001:2018 covers this, and an auditor needs to test it, not only read it. This guide is for auditors working to the standard and for anyone preparing for the CQI (Chartered Quality Institute) and IRCA (International Register of Certificated Auditors) ISO 45001 Lead Auditor course.

Quick answer

To audit emergency preparedness and response under ISO 45001, start from the emergencies the organization has identified through its hazard and risk assessment, then check the plan, the training, the testing and the communication. Review records, walk the site and ask workers, contractors and visitors what they would do. Check that drills and real events were evaluated and that the plan was revised where needed. Clause 8.2 of ISO 45001:2018 sets the requirements.

What does ISO 45001 require for emergency preparedness and response?

It requires processes to prepare for and respond to the potential emergencies the organization has identified.

In plain terms, clause 8.2 asks the organization to plan its response, including first aid, train people for it and test the response at suitable intervals. It also asks the organization to evaluate its performance and revise the plan when needed, especially after tests and after real emergencies. The organization must tell workers their duties and pass relevant information to contractors, visitors, emergency services, authorities and, where appropriate, the local community. It must also take into account the needs and capabilities of interested parties and keep documented information on the process and the plans. Always check the exact wording in the current standard. Our guide to how an ISO 45001 audit works shows where this check sits in an audit.

Which emergencies should the plan cover?

The plan should cover the emergencies that the organization’s own hazards and risks make possible.

There is no standard list, because the emergencies depend on the site. The organization identifies them through hazard identification and risk assessment, and the auditor checks that the plan matches. Our guide to risk assessment in ISO 45001 explains that starting point.

Possible emergencyExampleWhat the auditor checks
FireFire in a store room or workshopEvacuation routes, alarms and assembly points
Injury or illnessA serious cut or a collapse on the floorFirst aid provision, trained people and a call-out route
Hazardous releaseA chemical spill or gas leakSpill kits, isolation steps and trained responders
Equipment or structural failureA person trapped in machineryRescue arrangements and shutdown steps
Severe weather or power lossA storm that stops workShelter and communication arrangements

An emergency plan is only as good as the last time someone tested it.

How do you plan an audit of emergency preparedness?

Start from the organization’s identified emergencies, choose scenarios to sample and plan to cover every shift and every type of person on site.

Ask for the list of potential emergencies and the plan for each. Then pick a few scenarios, favoring the ones with the highest risk, and plan to visit the areas involved. Include different shifts, and think about contractors and visitors too, since they are on site but may not know the layout. Our ISO 45001 audit plan and checklist guide shows how to build these checks into your checklist, and our guide to the documents required for an ISO 45001 audit helps you decide what to read first.

What evidence should you collect?

Use all three methods: records for what was planned, observation for what exists and interviews for what people know.

RequirementRecordsObservation and interview
Planned responseEmergency plans and site layoutsWalk the routes and check the plan matches the site
First aidFirst aider lists and kit checksFind the kit and ask who the first aiders are
TrainingTraining and competence recordsAsk a worker what they would do if the alarm sounded
TestingDrill records and reportsAsk who took part and what happened
CommunicationInduction material and signageAsk a contractor or visitor how they were told

Remember that an interview comment is a lead, and you confirm it with a record or an observation. Our guide to how to interview workers in an ISO 45001 audit shows how to ask the questions, and our guide to competence and training in ISO 45001 covers the training records.

How do you test whether the emergency plan works?

Ask people what they would do, check the equipment and routes, and compare what you hear with the plan.

  1. Ask a worker what they would do if the alarm sounded right now, and where they would go.
  2. Walk the route and check that exits are clear, signs are visible and the assembly point exists.
  3. Check the equipment that the plan relies on, such as alarms, first aid kits and spill kits, and their inspection records.
  4. Ask about the last drill: who took part, what scenario it used and what went wrong.
  5. Compare the answers with the plan and the records, and note any difference.

Where the organization works with outside parties, our guide to how ISO 45001 manages contractors adds useful context, and our guide to worker consultation and participation in ISO 45001 explains why workers should help shape the plan.

Not sure whether to start as an internal or lead auditor? Compare the two roles in our guide.

Compare the roles

What do you look for after drills and real emergencies?

Look for evidence that the organization evaluated what happened and revised the plan where needed.

Clause 8.2 asks the organization to evaluate performance and revise its response after tests and, in particular, after real emergencies. A drill record that says “all OK” tells you little. A useful record shows who took part, how long it took, what problems appeared and what changed afterward. For a real event, look for the investigation and any corrective action. Our guide to incident investigation in ISO 45001 and our guide to verifying corrective action after an ISO 45001 audit show how those links work.

What gaps do auditors commonly find?

Common gaps are plans that do not match the site, drills that skip shifts and information that does not reach outside parties.

  • A plan that does not match the site. The layout or hazards changed but the plan did not.
  • Drills that skip shifts. Night or weekend workers have never taken part.
  • “All OK” drill records. Nothing is recorded about problems or changes.
  • Outside parties left out. Contractors and visitors do not know the arrangements.
  • Equipment not maintained. Kits, alarms or exits are not checked.

Treat these as prompts, not conclusions, and confirm each one with evidence before you raise a finding. For the wider skill set, see our guide on the skills you need to become an ISO 45001 auditor.

Emergency audit simulation: choose the best auditor action

For practice only. Read each situation and choose the best response. These scenarios were written by 3FOLD Training and are not real exam questions.

Situation 1. An organization shows you a fire evacuation plan and a drill record that says ‘all OK’ with no other details. What is the best action?
Model answer: A drill that took place is not the same as a drill that was evaluated. Ask who took part, what happened and what changed. Clause 8.2 asks the organization to evaluate its performance and revise the plan as needed.
Situation 2. Night-shift operators say they have never taken part in a drill. The drill records show drills on the day shift only. What do you do next?
Model answer: Verify before you conclude. Check attendance records and speak to workers on other shifts. If the gap is confirmed, you have evidence that the testing does not cover all workers.
Situation 3. A site hosts contractors and visitors every day. What is the best check?
Model answer: Clause 8.2 asks the organization to communicate relevant information to contractors and visitors. Check how it does this and ask some of them what they would do.
Situation 4. A chemical spill happened last month. What evidence of evaluation and revision do you look for?
Model answer: The standard asks the organization to evaluate its response and revise it, in particular after real emergencies. Look for the investigation, the changes made and the lessons shared.

1 scenario answered: 1 of 1 correct

Preloaded example: Situation 1 is already answered. A drill record that says “all OK” shows a drill took place, and the details show whether it taught the organization anything.

Frequently asked questions

What does ISO 45001 clause 8.2 require?

Clause 8.2 of ISO 45001:2018 requires the organization to establish processes to prepare for and respond to potential emergency situations. This includes a planned response with first aid, training, periodic testing, evaluation and revision of the response, and communication to workers, contractors, visitors and other relevant parties.

How do auditors audit emergency preparedness in ISO 45001?

Auditors start from the emergencies the organization has identified through hazard and risk assessment, then check the plan, training, testing and communication. They use records, observation of the site and interviews with workers, contractors and visitors, and they confirm what they hear with other evidence.

How often must emergency drills be done under ISO 45001?

ISO 45001:2018 clause 8.2 asks the organization to test and exercise its planned response periodically, but it does not set a fixed interval. The organization decides the frequency based on its risks, and legal requirements may add their own rules, so check what applies.

What evidence do auditors look for in ISO 45001 emergency preparedness?

Auditors look for emergency plans, training records, drill records showing what happened and what changed, equipment inspection records and evidence of communication to contractors and visitors. They also observe routes and equipment and ask workers what they would do.

Does ISO 45001 require first aid provision?

Yes. Clause 8.2 of ISO 45001:2018 includes the provision of first aid as part of the planned response to emergency situations. Auditors check that first aid arrangements match the risks of the site.

Does ISO 45001 emergency planning include contractors and visitors?

Yes. Clause 8.2 asks the organization to communicate relevant information to contractors, visitors, emergency response services, government authorities and, as appropriate, the local community. Auditors check how this information reaches outside parties.

What documented information does ISO 45001 require for emergency preparedness?

Clause 8.2 of ISO 45001:2018 requires the organization to retain documented information on the process and on the plans for responding to potential emergency situations. Auditors use these documents together with drill records and observation.
