When the alarm stops and people are safe, the work is not finished. What an organization does next shows whether its emergency arrangements really work. ISO 45001 expects the response to be evaluated, the plan to be revised where needed, the right people to be told and any resulting incident to be investigated. This guide sets out what should happen after an emergency under ISO 45001:2018 and how an auditor checks it. It is for auditors and for anyone preparing for the CQI (Chartered Quality Institute) and IRCA (International Register of Certificated Auditors) ISO 45001 Lead Auditor course. Always check the exact wording in the current standard.

Quick answer

After an emergency, ISO 45001 expects the organization to evaluate its response and revise the planned response as necessary, as clause 8.2 requires, particularly after real emergencies. It should communicate relevant information to workers, contractors, visitors and others, involve workers and relevant interested parties, and investigate any resulting incident, find the cause and take corrective action under clause 10.2. Records should show what was reviewed, decided and done.

What does ISO 45001 expect once an emergency is over?

It expects a review, a plan update, communication and an investigation where needed.

The four moves work as a loop. The organization evaluates how the response went, revises the plan where it fell short, tells the right people and investigates any incident that resulted. The aim is that the next emergency is handled better than the last. Our guide to auditing emergency preparedness and response in ISO 45001 covers the planning and readiness side, and our guide to the role of top management in ISO 45001 explains who is accountable.

Which ISO 45001 clauses apply after an emergency?

Clauses 8.2, 10.2, 7.4, 5.4 and 9.3 work together after an emergency.

ClauseWhat it asks after an emergency
8.2 Emergency preparedness and responseEvaluate performance and revise the planned response as necessary, including after emergencies
10.2 Incident, nonconformity and corrective actionReact, investigate, find causes and take corrective action
7.4 CommunicationCommunicate relevant information internally and externally
5.4 Consultation and participationInvolve workers in evaluating and improving the system
9.3 Management reviewReview incidents and results at top management level

An emergency plan that is never revised after a real event is only half a plan.

Want to practice auditing a real process? The course includes editable audit working documents and a case study.

See what’s included

How should the response be reviewed and the plan revised?

Hold a prompt review, record what went well and what did not, and update the plan.

  1. Review soon after while people still remember what happened.
  2. Compare what happened with what the plan said should happen.
  3. Record the gaps and what worked well.
  4. Assign actions with an owner and a date.
  5. Revise the plan and the training, then check that the changes work.

Our guide to auditing the hierarchy of controls helps when the review finds that stronger controls are needed, and our guide to auditing management of change in ISO 45001 shows how revised plans should be handled as a change.

Who must be told and involved after an emergency?

Workers, contractors, visitors and, where relevant, emergency services and authorities.

Clause 8.2 asks the organization to communicate relevant information on emergency preparedness to its workers, to contractors, visitors, emergency response services, authorities and, as appropriate, the local community. After an event, that includes what happened, what is changing and what people must do differently. Workers should also be consulted on the review, because they know what really happened on the floor. Our guide to worker consultation and participation in ISO 45001 explains how, and our guide to auditing contractor management in ISO 45001 covers contractors on site. Any reporting to authorities that the law requires is a legal matter, so check your own obligations.

When does an emergency become an incident to investigate?

When it caused harm or a near miss, it should be investigated and corrected under clause 10.2.

An emergency that led to injury, ill health, damage or a near miss is likely to be an incident. Clause 10.2 asks the organization to react, take action to control and correct it, evaluate the need for corrective action with workers involved, determine the root causes, review the effectiveness of the action and keep documented information. Injuries during evacuations and drills count too. Our guide to audit follow-up and corrective action in ISO 45001 shows how to check that actions really close the gap.

Not sure whether to start as an internal or lead auditor? Compare the two roles in our guide.

Compare the roles

Which records should you keep after an emergency?

Keep the report, the review, the actions, the revised plan and the communications.

Our guide to documents required for an ISO 45001 audit helps you decide what to read first, and our guide to auditing management review in ISO 45001 shows how results reach top management.

Which gaps do auditors find after emergencies?

Common gaps are no review, open actions, an outdated plan and workers left out.

  • No review. The emergency ended and nothing was recorded.
  • Open actions. Findings sit on a list for months with no owner.
  • Outdated plan. The plan was not revised or the new version was not shared.
  • Workers left out. People who were there were never asked.
  • Drill injuries ignored. Harm during a drill is not treated as an incident.

Treat these as prompts and confirm each one with evidence before you raise a finding. Our guide to continual improvement in ISO 45001 shows how lessons from emergencies feed improvement.

After-emergency simulation: choose the best action

For practice only. Read each situation and choose the best response. These scenarios were written by 3FOLD Training and are not real exam questions.

Situation 1. A fire evacuation took place last month. There is no record of any review afterwards. What is the best action?
Model answer: Clause 8.2 asks the organization to periodically evaluate its planned response and revise it as necessary, including after the occurrence of emergency situations. Ask how this is done and look for evidence.
Situation 2. The post-drill report says the assembly point was too small, but nothing has been done in six months. What do you do next?
Model answer: Finding a gap is only half the job. Look for an owner, a date and a check that the action worked, and ask how open actions are escalated.
Situation 3. After a chemical spill, the response was reviewed by managers only. Workers were not asked. What is the best action?
Model answer: Worker consultation and participation apply to the planning, evaluation and improvement of the system. Ask who was involved, and confirm with records and interviews.
Situation 4. A worker twisted an ankle during an evacuation. It was treated as part of the drill and not reported as an incident. What is the best action?
Model answer: Incidents are reacted to, investigated and corrected under clause 10.2. Ask how the organization decides what to investigate, and check whether the cause and the action were recorded.
Situation 5. The emergency plan was revised after an incident, but contractors on site still hold the old plan. What is the best action?
Model answer: The organization must communicate relevant information on emergency preparedness to workers, contractors, visitors and others. Look at how the revision was communicated and whether people hold the current version.

1 scenario answered: 1 of 1 correct

Preloaded example: Situation 1 is already answered. A fire evacuation should be followed by a review of the response.

Frequently asked questions

What should happen after an emergency under ISO 45001?

After an emergency, ISO 45001:2018 expects the organization to evaluate how it responded, revise its planned response where necessary, communicate relevant information, involve workers and others, and investigate any resulting incident and act on the causes. Always check the exact wording in the current standard.

Does ISO 45001 require a review after an emergency?

Yes. Clause 8.2 of ISO 45001:2018 asks the organization to periodically test and exercise its response capability and to evaluate performance and, as necessary, revise the planned response, including after testing and, in particular, after the occurrence of emergency situations.

Is an emergency an incident under ISO 45001?

It can be. An emergency that causes injury, ill health or a near miss is likely to be an incident, so the organization should react, investigate, determine the cause and take corrective action as clause 10.2 requires. The emergency response and the incident investigation work together.

What records should you keep after an emergency under ISO 45001?

Keep records such as the incident or emergency report, the post-event review, actions and their completion, plan revisions, communications to workers and others, and training or drill records. Clause 8.2 asks for documented information on the process and plans for responding to emergencies.

Should workers be involved after an emergency under ISO 45001?

Yes. ISO 45001:2018 asks for consultation and participation of workers in developing, planning, implementing and improving the system, and clause 8.2 asks the organization to ensure the involvement of relevant interested parties as appropriate. Workers can say what really happened and what to change.

How do auditors check what happens after an emergency in ISO 45001?

Auditors look at the review records, the actions raised, whether the plan was revised and communicated, and whether workers were involved. They interview people who took part and confirm the answers with records or observation.
