A lead auditor is more than the person who knows the standard best. The lead auditor is accountable for the whole audit: the plan, the team, the conduct, the findings and the report. Getting this role right is what makes an audit fair, reliable and useful to the organization. This guide explains the responsibilities of an ISO lead auditor, using the guidance in ISO 19011:2018 for auditing management systems, and it applies to audits of standards such as ISO 14001 and ISO 45001. Always check the exact wording in the current standards and the rules of the body you audit for.
An ISO lead auditor is responsible for leading the audit team and the whole audit. Before the audit, that means preparing the audit plan and briefing the team. During the audit, it means holding the opening and closing meetings, directing the team, collecting and verifying evidence, and managing problems as they arise. After the audit, it means reporting the findings and conclusions, agreeing follow-up and keeping the audit fair, impartial and confidential throughout.

What does an ISO lead auditor actually do?
Leads the team and takes responsibility for the audit from plan to report.
The lead auditor, called the audit team leader in ISO 19011, makes sure the audit meets its objectives and scope, that the team works to the plan and that findings are based on evidence. A lead auditor also decides what is reported and how, always within the rules of the audit program and the audit client. Our guide on ISO 45001 internal auditor vs lead auditor compares the roles.
Which principles guide the work of a lead auditor?
Integrity, fair presentation, due professional care, confidentiality, independence, an evidence-based approach and a risk-based approach.
| Principle | What it means for a lead auditor |
|---|---|
| Integrity | Act honestly, responsibly and competently |
| Fair presentation | Report truthfully and accurately, including obstacles and disagreements |
| Due professional care | Apply care and judgment in line with the importance of the task |
| Confidentiality | Protect the information gathered during the audit |
| Independence | Stay impartial and objective in findings and conclusions |
| Evidence-based approach | Base conclusions on verifiable evidence |
| Risk-based approach | Focus on what matters most for the objectives |
A lead auditor is accountable for the audit, and every conclusion must rest on evidence.

What are the lead auditor’s responsibilities before the audit?
Plan the audit, brief the team, assign work and prepare the documents.
- Confirm the objectives, scope and criteria with the audit client.
- Review the documents the organization sends and note any gaps.
- Prepare the audit plan with times, areas and people to see.
- Assign work to team members according to their competence.
- Brief the team on the plan, risks, rules and expected behavior.
Our guide to the ISO 45001 audit plan and checklist shows how a plan is built, and our guide to how an ISO 45001 audit works from start to finish shows the full cycle.
What are the lead auditor’s responsibilities during the audit?
Lead the meetings, direct the team, verify the evidence and manage problems.
- Opening meeting. Confirm the plan, the method and the contacts, and answer questions.
- Directing the team. Keep people on plan, review their progress and support them.
- Evidence. Make sure findings are based on records, observation and interviews.
- Problems. Resolve conflicts, and report significant obstacles to the audit client.
- Closing meeting. Present findings and conclusions clearly and fairly.
Our guide to how to interview workers and gather evidence in an ISO 45001 audit and our guide to how to conduct an ISO 14001 internal audit show the working methods.
What are the lead auditor’s responsibilities after the audit?
Report the results, agree follow-up and keep the information confidential.
After the audit, the lead auditor prepares and issues the audit report, makes sure it is accurate and complete, and communicates the conclusions to the audit client. Where the audit program requires it, the lead auditor also confirms the follow-up of corrective actions. The information gathered stays confidential unless the law or the audit program requires otherwise. Our guide to ISO 14001 nonconformity and corrective action covers what happens to the findings.

Which skills and behaviors does a lead auditor need?
Knowledge of the audit method and the standard, plus communication and leadership skills.
| Area | Why it matters |
|---|---|
| Audit method | Plans and evidence gathering follow a sound process |
| Management system standard | Findings are linked to the right requirement |
| Communication | Interviews and meetings are clear and respectful |
| Leadership | The team works as one and conflicts are managed |
| Judgment | Time and effort go to what matters most |
How is a lead auditor different from an internal auditor?
A lead auditor leads the team and the audit, while an internal auditor usually takes part in an audit.
The difference is accountability. An internal auditor performs audit tasks, while a lead auditor is responsible for the whole audit, for the team and for the report. A lead auditor course teaches the skills for that wider role. Becoming an IRCA graded Lead Auditor is a separate step that depends on audit experience, so see our guide to how to apply for CQI IRCA auditor grade membership. Frequently asked questions
What are the responsibilities of an ISO 45001 lead auditor?
An ISO 45001 lead auditor leads the audit team and is accountable for the whole audit of an occupational health and safety (OH&S) management system. Before the audit, the lead auditor prepares the plan and briefs the team. During the audit, they lead the meetings, direct the team and verify evidence. After the audit, they report the findings and conclusions and agree follow-up. This follows the guidance in ISO 19011:2018.
What principles apply to an ISO 45001 lead auditor?
ISO 19011:2018 lists integrity, fair presentation, due professional care, confidentiality, independence, an evidence-based approach and a risk-based approach. These apply to audits against ISO 45001, where findings must rest on verifiable evidence and be reported fairly.
What does an ISO 45001 lead auditor do before the audit?
Before an ISO 45001 audit, the lead auditor confirms the objectives, scope and criteria with the audit client, reviews the organization’s documents, prepares the audit plan, assigns work to team members according to their competence and briefs the team on the plan and the site safety rules.
What does an ISO 45001 lead auditor do during the audit?
During an ISO 45001 audit, the lead auditor holds the opening and closing meetings, directs the team, makes sure findings are based on records, observation and interviews with workers, resolves conflicts and reports significant obstacles to the audit client.
What does an ISO 45001 lead auditor do after the audit?
After an ISO 45001 audit, the lead auditor prepares and issues the audit report, makes sure it is accurate and complete, communicates the conclusions to the audit client and, where the audit program requires it, confirms the follow-up of corrective actions. The information gathered stays confidential unless the law or the audit program requires otherwise.
What is the difference between an ISO 45001 lead auditor and an internal auditor?
An ISO 45001 lead auditor leads the audit team and is accountable for the whole audit and the report, while an internal auditor usually performs audit tasks within a team or on their own. A lead auditor course teaches the skills for the wider leadership role.
Can an ISO 45001 lead auditor advise the organization on how to fix a finding?
A lead auditor can explain the ISO 45001 requirement and the evidence behind a finding, but should avoid designing the solution, which keeps the audit independent. The organization owns the corrective action.
What should an ISO 45001 lead auditor do if access to a work area is refused?
The lead auditor should discuss the reason with the organization, and follow any site safety rules, such as protective equipment or permits, that apply. If access is still refused, the lead auditor reports the significant obstacle to the audit client and records what was not covered. Auditors should not enter without permission.
Does the ISO 45001 Lead Auditor course teach these responsibilities?
According to the 3FOLD Training course page, the ISO 45001 Lead Auditor course is delivered as 40 hours of Virtual Instructor Led Training (VILT) and teaches the audit method used by auditors and team leaders. We prepare you for the exam; CQI and IRCA examine and certify.
Does completing the ISO 45001 Lead Auditor course make you an IRCA graded Lead Auditor?
No. The course gives you a certificate, and an IRCA auditor grade is a separate application that depends on the CQI and IRCA training and audit experience requirements. Check the current requirements with CQI and IRCA.
