How Do ISO 14001 Auditors Check That Corrective Action Worked?
Closing an action on paper is not the same as fixing the problem. Here is how auditors verify that a corrective action really worked and the nonconformity has not come back.

A form marked closed is not proof that anything changed. The real test of a corrective action is whether the problem stops happening. ISO 14001 builds this test into the standard, and auditors check it by looking again at the process, not just at the paperwork. This guide explains what the standard asks, how auditors verify effectiveness and what evidence convinces them. It is for managers running an environmental management system (EMS) and for anyone preparing for the CQI (Chartered Quality Institute) and IRCA (International Register of Certificated Auditors) ISO 14001 Lead Auditor course. Always check the exact wording in the current standard.
Under clause 10.2 of ISO 14001, the organization must review the effectiveness of any corrective action it takes. Auditors check this by confirming that the action was done, that it addressed the root cause, and that the problem has not recurred, using records, observation and interviews, and by looking at similar processes for the same weakness. They usually allow enough time for the process to run before they judge, and they expect records that show the check.
| Standard | ISO 14001 (Edition 4, 2026 replaces the 2015 edition) |
| Requirement | Clause 10.2: review the effectiveness of corrective action |
| Core test | Was the action done, did it address the cause, has the problem stayed fixed? |
| Evidence | Records, observation, interviews and later audit results |
| Key risk | An action closed on paper but not working in practice |

What does ISO 14001 require after a corrective action is taken?
It requires the organization to review whether the action was effective and to keep records.
Clause 10.2 of ISO 14001 sets out a sequence: react to the nonconformity, evaluate the need for action to remove the causes, implement the action, review its effectiveness, change the EMS if necessary and keep documented information. The effectiveness review is the step many organizations skip. Our guide to what a nonconformity is in ISO 14001 covers the definition, and our guide to ISO 14001 nonconformity and corrective action covers the whole process.
What does an effective corrective action mean?
It means the cause was removed and the problem has not come back.
| Question | Why it matters |
|---|---|
| Was the action carried out? | A plan that was never done fixes nothing |
| Did it address the root cause? | Fixing a symptom lets the problem return |
| Has the nonconformity recurred? | Recurrence shows the action did not work |
| Has anything worse appeared? | A fix can create a new problem elsewhere |
Closing an action on paper proves a form was filled in, not that the problem was fixed.

What do auditors look at first when they follow up?
They start with the original finding, the stated cause and the action plan.
- The original finding. What requirement was not met and what evidence showed it.
- The root cause. Whether the analysis went beyond the symptom.
- The action plan. Whether actions match the cause, with owners and dates.
- The records. Whether completion is shown with evidence, not just a signature.
Our guide to how to conduct an ISO 14001 internal audit shows how follow-up fits the audit cycle.
How do auditors test whether the action actually worked?
They go back to the process, sample new records and talk to the people doing the work.
- Re-check the original point and confirm the gap is closed.
- Sample new evidence created after the action, not only the fix itself.
- Observe the process in operation where possible.
- Ask the people who do the work what changed and why.
- Look for the same weakness in similar areas, sites or processes.
Our guide to operational control in ISO 14001:2026 explains how controls are checked, and our guide to ISO 14001 compliance evaluation helps when the finding concerned a legal obligation.
How long should an organization wait before judging effectiveness?
Long enough for the process to run several times, so recurrence would show.
The right gap depends on how often the process runs. A daily check can be judged after a few weeks, while an annual activity may need a later audit. The standard does not set a fixed period, so the organization should decide and record its reasoning, for example by setting a review date in the action plan. Certification bodies have their own rules for follow-up after a finding, so ask yours how it verifies. Our guide to internal audit in ISO 14001:2026 shows how follow-up can be scheduled.

What evidence convinces an auditor that it worked?
Dated records after the action, observation, trend data and a recorded effectiveness review.
| Check | Records | Interview or observation |
|---|---|---|
| Action completed | Action log with dates and evidence | Look at the change on site |
| Cause addressed | Root-cause record linked to the action | Ask how the action removes the cause |
| No recurrence | Records created after the action | Sample recent records and observe the task |
| Effectiveness reviewed | A recorded review with a date and a result | Ask who reviewed it and how |
| EMS updated | Revised procedure, risk or objective | Ask whether people use the new version |
Our guide to management review in ISO 14001:2026 shows how results should reach top management.
Which weak spots do auditors commonly find in corrective action?
Common weak spots are fixes that treat symptoms, no effectiveness review and repeat findings.
- Symptom only. The action fixes the instance but not the cause.
- Closed too soon. The action was closed before the process had run enough to show results.
- No review record. Nobody wrote down whether it worked.
- Repeat findings. The same nonconformity appears again at the next audit.
- No wider check. Similar areas were never checked for the same weakness.
Treat these as prompts and confirm each one with evidence before you raise a finding. ISO 14001:2026 is Edition 4 and replaces the 2015 edition. According to the CQI and IRCA summary of key updates, it adds planning of changes (6.3) and risks and opportunities (6.1.4), which affect how corrective changes are planned. Our guide to what is ISO 14001:2026 sets out the changes.

Frequently asked questions
How do ISO 14001 auditors check that corrective action worked?
Auditors confirm that the action was carried out, that it addressed the root cause and that the nonconformity has not recurred. They use records created after the action, observation of the process and interviews, and they look for the same weakness elsewhere. Always check the exact wording of clause 10.2 in the current standard.
What does ISO 14001 say about the effectiveness of corrective action?
Clause 10.2 asks the organization to review the effectiveness of any corrective action taken. It also asks for the organization to keep documented information as evidence of the nonconformities, the actions taken and the results.
What is a repeat nonconformity in ISO 14001?
A repeat nonconformity is the same or a very similar nonconformity found again after it was closed. It suggests that the earlier action did not remove the root cause, so auditors go back to the root-cause analysis.
Do certification bodies verify corrective action in ISO 14001 audits?
Certification bodies usually review the organization’s response and verify it under their own rules, which may include a document review or a follow-up visit depending on how the finding was graded. Ask your certification body how it verifies.
What is the difference between correction and corrective action in ISO 14001?
In ISO 14001, a correction deals with the nonconformity itself, for example fixing a missing record or replacing an empty spill kit. A corrective action removes the cause of the nonconformity so that it does not happen again. Clause 10.2 asks for both, as applicable.
Who is responsible for corrective action in an ISO 14001 environmental management system?
ISO 14001 asks top management to ensure that responsibilities and authorities for the EMS are assigned and communicated, so the organization decides who owns each corrective action. In practice the owner is usually the process manager, with top management reviewing the results at management review.
What records does ISO 14001 require for corrective action?
Clause 10.2 of ISO 14001 asks the organization to keep documented information as evidence of the nature of the nonconformities and any subsequent actions taken, and of the results of any corrective action. Always check the exact wording in the current standard.
